Hollywood Elementary has been chosen to pilot the electronic version of this form and we will be using this method to gather current emergency information for each of our students. HES parents/guardians will be able to access this form electronically through the Home Access Center (HAC). Using your own user name and password (not your student’s), parents/guardians can open the form, make changes, and submit the updated form online.

New School Year - Under Construction

Dear Parents and Guardians:
Greetings! I hope you all have had an enjoyable summer and are ready to start thinking about the new school year. We’ve been very busy at Hollywood Elementary this summer! Our renovation construction kicked off in July and our contractors are hard at work replacing our roof and HVAC system and improving our overall energy efficiency. As construction continues during the 2019/20 school year, you will notice that areas of the building, parking lots, and outside areas immediately surrounding the building will have limited use depending on the areas that are being worked upon. Safety is key for our students, staff, construction team as well as visitors to the building. We appreciate your cooperation and flexibility during this time! Thank you.
Open House for our Kindergarten through 5th-grade students and families will be on Thursday, August 29. Visit the school any time between 2:00 and 3:30pm. Students will find out their homeroom teacher assignments and have an opportunity to meet their teacher and drop off school supplies. Parents/Guardians can get bus information, HAC logins and passwords, and apply to be an SMCPS volunteer. Please remember that this is not a time to conference with teachers. Parent/teacher conferences will be held in October. Parents/Guardians may also email their student’s teacher to communicate more immediate concerns or questions.
